Refocus Medical brings personalized, relationship-centered geriatric primary care directly to you or someone you love. No waiting rooms. No rushed appointments. Just honest conversations, thoughtful guidance, and care that feels personal again.
Care for older adults should not feel rushed, fragmented, or impersonal.
I founded Refocus Medical because I needed to refocus on what mattered most to me as a clinician — and that meant returning to the kind of care I have always believed in. Care that listens closely, notices what has changed, understands the home and the people involved, and stays honest about what is realistic, what matters, and what comes next.
Many older adults are living with chronic illness, memory changes, medication complexity, falls, recent hospitalizations, or care needs that shift over time. That kind of care can't happen in a rushed fifteen-minute visit — and you shouldn't have to settle for one.
Refocus Medical brings primary care directly to older adults, wherever they live, with thoughtful medical care, clear communication, and honest conversations at the center. If that's the kind of care you've been looking for — for yourself or someone you love — we'd be glad to be part of your story.
Refocus Medical looks at care through three important lenses: the setting, the changes between visits, and the relationship over time.
Care looks different at home, in assisted living, and in memory care. We consider the environment, caregiver support, facility needs, safety concerns, routines, and what is actually feasible before making a plan.
For patients who would benefit from closer follow-up, remote patient monitoring allows certain health readings — such as blood pressure, weight, or oxygen levels — to be shared from home. This helps us notice changes sooner and stay more connected between visits.
You get to know your provider, and your provider gets to know you. Over time, that relationship helps care feel more personal, practical, and grounded in what matters most.
For patients who are most comfortable, and best understood, right where they live.
Working alongside communities that want thoughtful, relationship-based care for residents.
Honest, patient communication for families and care teams navigating memory changes.
We may be a good fit for patients and families when getting to appointments has become difficult, care feels more complicated, or everyone needs a clearer understanding of what's happening. Families often call when a loved one is having more falls, more confusion, more medication changes, a recent hospital or rehab stay, or a general sense that something is changing and they need guidance.
We also work alongside assisted living and memory care communities that want thoughtful, relationship-based medical care for their residents. Our goal is to support the patient, the family, and the care team with communication that's clear, practical, and honest.
Refocus Medical is not a replacement for emergency care, hospital-level care, daily caregiving, or private duty nursing. Our role is ongoing primary care, care coordination, and medical guidance for older adults whose needs are best understood in the place they live.
Contact us through this site or by phone. We'll have a brief conversation to understand your needs, location, care setting, and current insurance or payment preference.
If Refocus Medical is the right fit, we make the next steps simple. New patient consents and enrollment forms are sent electronically, so you or your loved one can review and complete them from home.
Once intake is complete, your provider will reach out to schedule the first visit. We come directly to you or your loved one — at home, in assisted living, or in memory care — for thoughtful care in a familiar place.
After the first visit, we build a care plan around what is actually needed. We continue to support the patient, family, and care team with primary care, medication review, care coordination, and honest communication as needs change.
Refocus Medical provides primary care for older adults in the comfort and familiarity of their own environment. From routine wellness to complex chronic disease management, we bring thoughtful medical care into the place where life is actually happening.
Routine and acute care visits in the home. Annual wellness exams, chronic disease management, medication reviews, and preventive care — all without leaving home.
We communicate directly with families and caregivers, giving honest, clear updates on their loved one's condition, treatment options, and what to realistically expect.
Ongoing, personalized management of complex conditions including heart disease, diabetes, COPD, dementia, and more — with a focus on quality of life, not just numbers.
Honest, compassionate conversations about what matters most — advance care planning, changes we're seeing, likely outcomes, and care options — held with patience and without agenda.
Careful follow-up after hospital or facility discharge — the critical window where patients are most vulnerable and most in need of attentive, coordinated care.
All patients have direct access to their provider by phone or message — not a call center, not a portal. Real access to the person who knows your loved one's care when questions arise.
Continuous monitoring of vitals and health data between visits — keeping a close eye on your loved one's condition from a distance so issues are caught early, not late.
Billed through your insurance plan when Refocus Medical is contracted with your plan.
Visit fees or applicable charges may be billed separately depending on coverage and plan status.
Remote patient monitoring availability depends on clinical appropriateness, device needs, and patient participation.

"I started Refocus Medical because I needed to refocus on my purpose. When I got honest with myself about what mattered, I couldn't go back to practicing any other way."
For years, I've cared for older adults where life is actually happening — in their homes, assisted living communities, and memory care settings.
I've seen how much good care can happen when a provider slows down enough to listen, notice what has changed, and understand the full picture. When families have room to ask hard questions. When patients are seen as whole people, not just diagnoses. When the care plan reflects the reality of daily life.
Refocus Medical was created to return to that kind of care.
Honest, relationship-based care for older adults — right where they live.
I'm Camelia Martin, DNP, FNP-BC, a board-certified family nurse practitioner focused on caring for older adults with honesty, clarity, and compassion.
I completed my Doctor of Nursing Practice at the University of Arizona, where my clinical focus was palliative care and symptom management. My experience includes geriatric medicine, rehabilitation, long-term care, palliative care, and hospice.
Those years have shaped how I care for people now — by looking beyond the diagnosis, listening closely, and helping patients and families understand what is happening, what matters, and what comes next.
Outside of work, I'm happiest spending time with my husband and my son, traveling, and being in nature. Whether we are snowboarding, kayaking, off-roading, or exploring somewhere new, I value time with family, fresh air, and a little adventure.
Whether you're ready to become a patient or just want to understand if Refocus Medical is the right fit for your family — reach out. We'll have a real conversation, answer your questions honestly, and help you figure out next steps.
